About The Position

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. The role delivers nursing services per Hospice and Palliative care standards including providing world-class customer service to patients, providers, service lines, outpatient and inpatient departments and other health systems.

Requirements

• Associate’s degree or equivalent, Graduate from an accredited nursing program
• 3 years of relevant experience in Nursing
• Michigan Registered Nurse (RN) license upon hire

Responsibilities

• Manages and documents calls in a timely, efficient manner consistent with Hospice and Palliative Care standards.
• Contributes to meeting service level commitments by handling calls in a manner that supports overall department goals.
• Documents the patient history, diagnosis, medications and plan of care in electronic health record and completes any additional documentation as appropriate.
• Obtains pertinent information in order to make the best decisions in referring to appropriate level of care and patient placement.
• Communicates with physicians/nurses/other staff and provider office staff as necessary.
• Connects customers to the appropriate person(s)/resources.
• Collaborates with physicians, nurses, care managers, midlevel providers and outpatient and inpatient leadership teams to facilitate provision of care to patients and others with symptoms, referrals or other clinical questions.
• Collaborates with physician for certification of eligibility.
• Actively participates and contributes to a culture of quality, safety and collaboration that embodies and represents Corewell Health's core values of Excellence, Accountability, Compassion, Integrity, Respect, and Teamwork.

• Educates potential patient/families in the Hospice at Home approach to end-of-life care.
• Identifies the patient and families most immediate needs.
• Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
• Performs other duties as assigned.
