About The Position

This position is for a Hospice Intake/Triage Nurse, a partial remote role for local candidates only. The role involves coordinating referrals and physician orders for patients discharging to Hospice services, facilitating communication between staff, referral sources, insurance companies, and physicians. The nurse will perform phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to patient care, providing clinical support to the hospice team by assisting in the coordination of patient care services.

Requirements

• Specialized Diploma NLN approved/accredited nursing program
• 1 year nursing experience in a healthcare setting and 1 year direct nursing care experience
• Demonstrates knowledge and ability to care for patients based on the population served

Nice To Haves

• Associate’s Degree Nursing or Bachelor’s Degree Nursing
• 1 year home health/hospice experience or Less than 1 year nursing experience in a clinical office setting or Less than 1 year nursing experience performing phone triage
• CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years

Responsibilities

• Collects data and evaluates patient appropriateness for Hospice Services through prescreening and communication with healthcare team members, and facilitates patient transition to appropriate level of hospice care.
• Triages patient/caregiver phone calls utilizing critical thinking skills and sound clinical judgement.
• Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.
• Provides hand-off communication to members of the care team as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or a phone call.
