LV HOME CARE AND HOSPICE HOSPICE INTAKE/TRIAGE NURSE - WEEKENDS Summary Coordinates referrals and physician orders for patients discharging to Hospice services. Facilitates communication between staff, referral sources, insurance companies, and physicians. Performs phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care. Provides clinical support to the hospice team by assisting in the coordination of patient care services.

Requirements

• Specialized Diploma NLN approved/accredited nursing program
• 1 year nursing experience in a healthcare setting and 1 year direct nursing care experience
• Demonstrates knowledge and ability to care for patients based on the population served
• Demonstrates critical thinking skills and sound clinical judgement
• Demonstrates analytical and problem-solving skills
• Ability to perform duties with accuracy and attention to detail

Nice To Haves

• Associate’s Degree Nursing or Bachelor’s Degree Nursing
• 1 year home health/hospice experience or Less than 1 year nursing experience in a clinical office setting or Less than 1 year nursing experience performing phone triage
• CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years

Responsibilities

• Collects data and evaluates patient appropriateness for Hospice Services through prescreening and communication with healthcare team members, and facilitates patient transition to appropriate level of hospice care.
• Triages patient/caregiver phone calls utilizing critical thinking skills and sound clinical judgement.
• Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.
• Provides hand-off communication to members of the care team as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or a phone call.
