LV HOME CARE AND HOSPICE TRIAGE NURSE - HOSPICE - FULL TIME NIGHTS Summary Provides clinical support to the clinical services team by assisting in the coordination of the delivery of patient care services. Performs phone triage, data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care.

Requirements

  • Specialized Diploma NLN approved/accredited nursing program
  • 1 year nursing experience in a healthcare setting and 1 year direct care nursing experience
  • Demonstrates knowledge and ability to care for patients based on the population served
  • Demonstrates critical thinking skills and sound clinical judgement
  • Demonstrates analytical and problem-solving skills
  • Ability to perform duties with accuracy and attention to detail
  • Demonstrates excellent verbal and written communication skills

Nice To Haves

  • Associate’s Degree Nursing or Bachelor’s Degree Nursing
  • 1 year home health/hospice experience and Less than 1 year nursing experience in a clinical office setting or Less than 1 year nursing experience performing phone triage/on call responsibilities
  • AMB-BC- Ambulatory Care Nursing-Board Certified - American Nurses Credentialing Center within 3 Years or CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years

Responsibilities

  • Triages patients through phone calls utilizing critical thinking skills and sound clinical judgement
  • Performs data/order entry, patient/staff scheduling, and other clinical tasks related to the provision of patient care
  • Provides hand-off communication to members of the care team as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.
