About The Position
RN Case Manager – Hospice (The Villages, FL) Compassionate Care Hospice, an Amedisys Company. This position is based in The Villages, FL and requires daily travel to assigned area. Schedule: Monday-Friday, 8:00 AM - 5:00 PM. At Compassionate Care Hospice, we go above and beyond—not only for our patients but for each other. Compassion, collaboration, and commitment are at the heart of everything we do. Salary Range: $82,000–$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.
Requirements
- Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting (Required)
- Current CPR certification

- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
- Records pain/symptom management changes/outcomes as appropriate.
- Teaches patients, families, and caregivers appropriate patient care techniques.
- Regularly communicates with clinical manager and care team.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Other duties as assigned.
