About The Position

A caregiver in this role will serve patients in the following cities: Cleveland, Parma, Brook Park, Old Brooklyn, Lakewood, Parma Heights and others as needed. Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, you are responsible for providing direct nursing care, participating in a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit. Due to continuously working with each patient, Homecare RN’s have more acute findings for patients and are easily able to identify changes. A caregiver in this position works days from 8:00AM – 5:00PM or 8:00AM – 6:00PM, with on-call requirements.

Requirements

  • Graduate from an accredited school of Professional Nursing (Diploma, ADN or BSN program)
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
  • Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
  • Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
  • One year of current clinical experience as an RN
  • One year of experience in an acute Medical/Surgical or Home Care setting

Responsibilities

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
  • Implement and monitor patient care plans.
  • Provide supervision to LPNs, home care aides, family and caregivers involved in the care of the patient.
  • Perform accurate and complete admission assessments and reassessments within specified time frame.
  • Assess patients for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patient, family and health care team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.
