About The Position

This job provides home visiting to all families with newborns in a specified geographical area. It supports and assesses family needs in 12 domains of physical and psychosocial well-being and facilitates referrals and connections to community agencies to support families in these areas. The role works in a team environment of clinical and non-clinical staff to schedule, visit, and support families in the community using an evidence-based home visiting protocol. Requirements

  • Associate’s degree in nursing.
  • 1 year of maternal/child and home visiting experience.
  • Registered nurse (RN) license in state of practice.
  • BLS certification from American Heart Association.
  • Valid driver’s license in state of practice.
  • Clinical skills and ability to appraise situations and make decisions.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally and work effectively with diverse patient populations.
  • Ability to work with the nurse supervisor to implement practice improvement and clinical development plans.
  • Interpersonal skills and ability to work in a team of clinical and non-clinical staff.
  • Effective time-management, organization, and multitasking skills.

Nice To Haves

  • Bachelor’s degree in nursing.
  • Experience in lactation education, domestic violence, substance abuse/depression, social work, or related field.
  • Certified Lactation Educator or Consultant.
  • Fluency in Spanish.

Responsibilities

  • Follows home visiting protocol, keeps records of client observations and activities, documents information for family assessments and program evaluation, and records information in real time via in-home use of computerized record system.
  • Follows scheduling protocol and visits target area hospital to schedule home visits for new mothers prior to discharge from the hospital.
  • Provides a health and physical assessment of mother and baby in the home to determine well-being and assesses family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence and appraises situations and makes decisions.
  • Establishes a trusting relationship with families during in-home visits and engages the family to establish a support plan with goals, objectives, and activities that address family strengths and needs.
  • Works with the team to ensure appropriate referrals and connections to community resources and to assist parents in making and attending appointments for doctors, social service agencies, etc.
  • Acts as a liaison between families and other community agencies and serves as family advocate.
  • Maintains eight open visit slots, conducts and documents at least six home visits per week, responds to client needs through flexible scheduling of home visits, including occasional home visits during evening or weekend hours, and documents and sends letters within 48 working hours of the visit.
  • Attends weekly team, clinical supervision, staff, and other meetings and supports and encourages development of staff and programs.
  • Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Performs other related duties as assigned.
  • Remains knowledgeable on current federal, state, and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations, and standards.
  • Complies with the Compliance & Privacy Program and Standards of Conduct, including the immediate reporting of any known or suspected unethical or questionable behaviors or conduct; patient/employee safety, patient privacy, and/or other compliance-related concerns.
