About The Position

Welcome Family Springfield Holyoke is seeking a part-time Maternal Child Health Nurse to provide universal, one-time home visits to new mothers and their newborns in Springfield, Holyoke, and surrounding communities. These visits, lasting up to 90 minutes, can be conducted in-person or virtually, up to 8 weeks postpartum. The role involves collaboration with referral sources, outreach to potential participants, and conducting comprehensive screenings in areas such as clinical assessment, nutrition, substance use, emotional health, and domestic violence. The nurse will provide education, address caregiver concerns, offer referrals to community resources, and ensure culturally appropriate engagement. Data collection, documentation, and participation in training and collaborative learning are also key aspects of the position. Welcome Family partners with Baystate's Babies at the Wesson Women & Infants’ Unit.

Requirements

  • Current licensure as an RN by the Massachusetts Board of Registration, Division of Professional Licensure
  • Bachelor's Degree in Nursing or Master of Science Degree in Nursing in Maternal and Child Health, Family Health, Community Health, or related specialty
  • Two or more years of professional work experience with prenatal, newborns, pediatrics, lactation, or maternal services
  • CLC Certification required

Responsibilities

  • Collaborate with referral sources
  • Provide outreach to potential participants
  • Schedule and conduct up to 90-minute visits at the family's home or by telehealth
  • Screen in six core areas: clinical assessment, unmet health needs, maternal and infant nutrition, substance use, emotional health, and domestic violence
  • Provide brief intervention and education to all families, regardless of perceived risk or need
  • Address the caregiver's individual questions and concerns
  • Offer referrals to services for caregivers with an identified need, including MA MIECHV-funded home visiting programs and other community resources that may best suit the needs of the family
  • Engage, assess, and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topics
  • Conduct a follow up contact with all caregivers 2-3 weeks post initial visit
  • Collect data using the Welcome Family forms, complete required DPH and agency documentation and billing
  • Participate in the Welcome Family Learning Collaborative and DPH annual site visit
  • Complete agency and state required training
