About The Position

This role is for a Registered Nurse (RN) in Home Health, working under the supervision of the Nurse Manager and Director. The RN is responsible for the comprehensive nursing care of assigned patients on an intermittent visiting basis, typically in a home care setting. This involves assessment, planning, implementation, and evaluation of patient care, utilizing communication, critical thinking, and problem-solving skills to develop individualized care plans that consider physiological, psychosocial, and cultural diversity. The RN also supervises and delegates tasks to non-professional staff to ensure optimal patient care and must demonstrate competence in age-related needs of specific patient populations. Collaboration with a multidisciplinary team, including physical, speech, and occupational therapy, medical social services, and Home Health Aides, is essential to provide comprehensive care in accordance with Riverside Health System's standards and goals.

  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)

Responsibilities

  • Assesses, plans, implements, and evaluates the total nursing care of assigned patients on caseload on an intermittent visiting basis.
  • Applies standards of practice as it relates to the specialty or patient population served (e.g., med surg, hospice, IV therapy).
  • Develops the initial and ongoing comprehensive assessment of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.
  • Supervises and delegates non-professional staff to ensure optimum patient care.
  • Recognizes and responds to emergency situations.
  • Evaluates own needs for support and uses identified systems and resources to meet patient care needs.
  • Supervises LPN/LVNs, and Home Health Aides in providing services.
  • Completes all required clinical documentation consistently per RHS standards, including completing and "locking" patient notes and verbal orders, as well as completing certifications and recertifications after the department's multi-disciplinary team meetings.
  • Completes required documentation no later than the last day of the month to provide timely billing.
  • Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, counsels the client and family in meeting nursing needs, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from services.
  • Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.
  • Participates in case conferences.
  • Provides education to client and family.
  • Participates in training and support of all team members, volunteers, and students as needed.
