About The Position
The RN Home Health position is responsible for the assessment, planning, implementation, and evaluation of the total nursing care of assigned patients on an intermittent visiting basis. This role typically operates in a home care setting and requires the utilization of communication, critical thinking, problem-solving, and decision-making skills to create individualized care plans. The RN will also supervise and delegate tasks to non-professional personnel to ensure optimal patient care and must demonstrate competence in addressing age-related needs of diverse patient populations. Collaboration with a multidisciplinary team, including therapy services, medical social services, and Home Health Aides, is essential to provide comprehensive care in accordance with Riverside Health System's standards and goals.

- Registered Nurse (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Driver's License (Required)

Responsibilities
- Assesses, plans, implements, and evaluates the total nursing care of assigned patients on caseload on an intermittent visiting basis.
- Applies standards of practice as it relates to the specialty or patient population served (e.g., med surg, hospice, IV therapy).
- Develops the initial and ongoing comprehensive assessment of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.
- Supervises and delegates non-professional staff to ensure optimum patient care.
- Recognizes and responds to emergency situations.
- Evaluates own needs for support and uses identified systems and resources to meet patient care needs.
- Supervises LPN/LVNs, and Home Health Aides in providing services.
- Completes all required clinical documentation consistently per RHS standards, including completing and 'locking' patient notes and verbal orders, as well as completing certifications and recertifications after department multi-disciplinary team meetings.
- Completes required documentation no later than the last day of the month to provide timely billing.
- Implements/initiates a plan of care and revises when necessary.
- Provides services and/or treatments requiring specialized nursing skill.
- Councels the client and family in meeting nursing needs.
- Coordinates services.
- Informs the physician and other staff of changes in the client's needs.
- Plans for the patient to be discharged from services.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.
- Participates in case conferences.
- Provides education to client and family.
- Participates in training and support of all team members, volunteers, and students as needed.
