About The Position

Horizon Home Healthcare LLC is seeking a compassionate and experienced RN to join their home health team in Montgomery County, Ohio and surrounding areas. This role involves managing patient care from admission through discharge, completing OASIS assessments, developing individualized care plans, and collaborating with an interdisciplinary team to achieve optimal patient outcomes. The position offers autonomy, flexibility, and the opportunity to build meaningful patient relationships.

Requirements

Current Ohio RN license
Current CPR and/or BLS certification
Valid driver's license, reliable transportation, and proof of auto insurance
Strong assessment, communication, organizational and time management skills

Nice To Haves

Experience in home health and OASIS assessments
Knowledge of Medicare home health regulations and documentation requirements

Responsibilities

Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments
Conducts comprehensive nursing assessments and develop individualized plans of care
Coordinates and manages patient care throughout the episode of care
Provides skilled nursing services, patient education, medication management, and disease management
Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team
Monitors patient progress and adjust care plans as needed
Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines
Participates in quality improvements initiatives and maintains compliance with regulatory standards
