About The Position
The RN in Home Health is responsible for delivering care to patients across multiple counties, utilizing Homecare Homebase for charting. The role requires experience in home health and proficiency with Homecare Homebase charting. The RN will manage approximately 30 adult patients, ensuring start of care, recerts, and daily visits according to the point system.
Requirements
- Valid RN License in the State of work
- Minimum of 1 year of experience on a Medsurg floor in a hospital setting
- Experience with Homecare Homebase charting
- BLS Certification
Responsibilities
- Provide care to patients in a home health setting across specified counties
- Utilize Homecare Homebase charting system
- Case manage approximately 30 patients
- Ensure completion of 6.5 points per day, including start of care and recerts
