About The Position

This role involves providing comprehensive home health care services. The Registered Nurse (RN) will conduct initial assessments of patients and their families to identify home care needs, performing complete physical assessments and reviewing medical histories. The RN will continuously re-evaluate patient nursing needs, initiate and revise care plans based on changes in patient status, and utilize health assessment data to establish nursing diagnoses. The position requires developing care plans with goals that incorporate therapeutic, preventive, and rehabilitative nursing actions, involving the patient and family in the planning process. The RN will also initiate preventive and rehabilitative procedures, administer medications and treatments as prescribed, and counsel patients and families on meeting their health needs. Health care instructions will be provided as appropriate, and discharge planning needs will be identified and implemented. When assigned as Case Manager, the RN will coordinate patient care for their caseload. Communication is key, involving preparing clinical notes, updating physicians regularly (at least every sixty days), and communicating any changes in patient condition or needs. The RN will also communicate with other community health professionals to coordinate care plans. Additional duties include participating in on-call duties, ensuring necessary equipment and services are available, and instructing, supervising, and evaluating home health aide care every two weeks.

Requirements

Registered Nurse (RN) license
Ability to perform complete physical assessments and medical histories
Proficiency in developing and implementing care plans
Knowledge of nursing diagnoses and therapeutic, preventive, and rehabilitative nursing actions
Skill in administering medications and treatments
Ability to counsel patients and families
Understanding of discharge planning
Case management skills
Strong communication skills for updating physicians and coordinating with health professionals
Ability to participate in on-call duties
Capability to ensure availability of necessary equipment and services
Experience in instructing, supervising, and evaluating home health aides

Responsibilities

Completes an initial assessment of patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es).
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan of care.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
Communicates with community health related persons to coordinate the care plan.
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
