About The Position
The Care Manager RN - Home Health will be responsible for managing patient care within a home health setting. This role involves a day shift schedule, Monday through Friday, from 8 am to 5 pm, with rotating weekends as part of the 40-hour work week. The position requires EPIC experience and the flexibility to provide coverage in Glendale when necessary.
Requirements
- Registered Nurse (RN) licensure in the state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification from approved vendor per AH policy
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Current automobile insurance and a reliable automobile
- EPIC experience required
