About The Position

As the RN Home Health Care Manager, you'll have the opportunity to lead a compassionate team dedicated to providing exceptional care where patients are most comfortable, at home. You'll inspire and support nurses, therapists, and clinicians while ensuring every patient receives safe, high-quality, and personalized care throughout their home health journey.

Requirements

  • Current Louisiana Registered Nurse (RN) license.
  • Current American Heart Association BLS Certification.
  • Minimum of five (5) years of professional nursing experience.
  • Strong knowledge of Medicare regulations, OASIS documentation, Conditions of Participation, and home health compliance standards.
  • Demonstrated leadership, communication, problem-solving, and team-building skills.
  • Ability to lead quality improvement initiatives while balancing patient care excellence with operational and financial performance.

Nice To Haves

  • Bachelor's degree in Nursing (BSN) preferred.
  • Previous Health Home Care, Home Health, Case Management, Clinical Supervisor, or Nursing Leadership experience preferred.

Responsibilities

  • Lead the daily clinical and operational activities of the Health Home Care department.
  • Supervise and support a multidisciplinary team of Nurses, therapists, and other staff providing care in patients' homes.
  • Ensure delivery of safe, evidence-based, patient-centered home health services.
  • Coordinate and participate in ongoing OASIS training for staff.
  • Manage staffing, scheduling, hiring, onboarding, performance evaluations, coaching, and employee development.
  • Maintain accountability for agency policies, nursing standards, Medicare Conditions of Participation, state regulations, and accreditation requirements.
  • Partner with physicians, case managers, referral sources, and healthcare leaders to coordinate seamless transitions of care.
  • Monitor quality metrics, patient outcomes, and satisfaction scores while leading quality assurance and performance improvement initiatives.
  • Promote patient safety by maintaining a culture of accountability and ensuring timely completion and follow-up of employee and patient safety reports.
  • Participate in budget planning while managing department productivity, resource utilization, and cost containment initiatives.
  • Implement evidence-based practices and continuously evaluate opportunities to improve clinical outcomes and operational efficiency.
  • Develop, review, and update departmental policies and procedures to ensure regulatory compliance and best practices.
  • Foster a collaborative, positive work environment that supports employee engagement, professional growth, and exceptional patient experiences.
  • Maintain strong working relationships with organizational leaders and participate on interdisciplinary committees to support strategic initiatives and safe, efficient patient care.
