About The Position

This position assesses the needs of patients and families in the home environment, develops a plan of care in conjunction with the physician and other health care providers, and administers prescribed care according to physician orders. The role also involves educating caregivers on the plan of care, providing support to patients and families, and working with support services to obtain necessary supplies, equipment, and services for patients.

Requirements

Work requires a basic knowledge of pathophysiology, the nursing process, nursing interventions, pharmacology, methods utilized for delivering nursing care, and the management of patients and personnel.
Ability to complete documentation on a computer.
Graduation from an accredited school of nursing and current Indiana RN license required.
Current Indiana Driver’s License required.
Must have own car.
Must pass Criminal Record Check.
Must be free of infectious disease.

Nice To Haves

For Home Care RN, six (6) months of recent nursing experience and previous home care experience preferred.
For Hospice RN, one (1) year nursing experience and previous hospice experience preferred.
