About The Position

Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years, serving all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic, self-motivated RN Case Manager Float - Palliative/Hospice Care, who thrives in a fast-paced environment and supports our mission.

Requirements

  • Possess reliable transportation and have a valid California driver’s license and have automobile insurance which meets State standards.
  • All employees who drive need to provide Hospice of San Joaquin with evidence of insurance.
  • Possess a license to practice as a registered nurse in the State of California.

Responsibilities

  • Graduate of an accredited school of nursing in the State of California as a Registered Nurse.
  • Three years of recent clinical experience.
  • Computer skills.
  • Ability to learn and assimilate the clinical computer program system.
  • Knowledge of advancing chronic or life-limiting illness/conditions, signs/symptoms, treatment interventions, and medication for palliative care.
  • Ability to handle multiple projects and tasks simultaneously; uses time and resources efficiently to accomplish work objectives.
  • Ability to work in a high-stress environment or with participants who are under stress and maintain a positive work attitude while working with customers/participants, staff, and the community.
  • Excellent verbal and written communication skills.
  • Ability to analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations.
  • Ability to write reports.
  • Ability to effectively present information and respond to questions from groups or managers, clients, customers, and the general public.
  • Demonstrated ability to write in a clear, understandable, and concise manner.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent.
  • Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
