About The Position

The RN Float Nurse has the responsibility of providing direct and indirect nursing care to assigned patients across sites (Gastonia and Shelby). Works under the direct supervision of the Clinical Services Director but is independent in the application of basic nursing knowledge and skills.

Requirements

  • Graduate of an accredited school of nursing.
  • Has practiced nursing in the last 3 years.
  • Has 3 years of RN experience (or a combination of RN and LPN experience) with at least 1 year of experience working with the frail, elderly population.
  • RN licensed in NC.
  • Meet a standardized set of competencies for the specific position description established by Senior TLC, Inc. and approved by CMS before working independently.
  • Must possess knowledge and skills necessary to treat patients; other training as required by department.
  • Ability to practice at the top of his or her license but only act within the scope of his or her authority to practice.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Excellent organizational and time management skills.
  • Strong case management background.
  • Strong ability to multitask.
  • Sort through complex issues and conducts analysis of multiple solutions.
  • Ability to work as part of an interdisciplinary team.
  • Able to influence others.

Responsibilities

  • Collect patient data and follow up with appropriate responses according to unit standards.
  • Identifies patient’s overt problems/needs and sets priorities.
  • Identifies problems requiring further referral and/or follow-up.
  • Will observe any participants to ascertain if the participant needs to be seen in the clinic.
  • Identifies emergency situations and independently initiates advanced therapy.
  • Ability to complete a head-to-toe comprehensive assessment and document appropriately.
  • Ability to use critical thinking skills.
  • Identifies and monitors acute participant changes and communicates participant changes to providers in a timely and effective manner.
  • Administers medications, provides the participant with training in self-administration of all non-scheduled medications, and monitors medication compliance.
  • Demonstrates understanding of age-related characteristics and needs of patients served.
  • Implements and monitors plan of care, identifying and communicating opportunities for improvement.
  • Implement nursing measures related to impending or associated problems.
  • Demonstrates the ability to provide preventative/proactive and reactive care.
  • Works well independently and takes charge of areas of responsibilities.
  • Participate in the on-call rotation for patient response after hours to continue coordination of 24-hour care.
  • Communicates with participants, providers, and the interdisciplinary team to ensure team and care givers are notified of participant care and/or changes as appropriate.
  • Monitors participant attendance and follows up on unexplained absences and forwards the information to the appropriate care team.
  • Explains nursing procedures in an understandable manner.
  • Recognizes inappropriate and/or ineffective participant care management and reports this to the interdisciplinary team.
  • Demonstrates the ability to delegate appropriate tasks to clinical support staff.
  • Ensures delegated tasks are completed in a timely manner.
  • Provides clinical teaching and clinical supervision of STLC Day Center Staff.
  • Comply with policies addressing safe working conditions.
  • Maintains confidentiality of information by adhering to HIPPA laws, rules, and regulations.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations, adhering to infection-control policies and protocols.
  • Complies with all company safety and risk management policies and procedures.
  • Attends required training programs (classroom and virtual), to include computer training.
  • Reports all accidents and injuries in a timely manner.
  • Additional work functions and duties that may be assigned.
