About The Position

This is a flex nursing role which provides case management, home health and clinic visits in Napa county. As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The Flex RN position supports and provides coverage for the PACE RN Case Manager position and PACE clinic nurse and home health nurse positions. This dynamic position utilizes the full-spectrum of high level nursing skills-- from critical -thinking care coordination and triage skills, to hands-on, clinical nursing skills. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Requirements

• Education sufficient for licensure
• National Provider BLS - American Heart Association (within 30 days of hire)
• California Registered Nurse License (upon hire)
• 1 year Long term care or geriatric nursing
• 1 year Working with the frail and elderly
