RN Field Supervisor - Licensed Home Care - Per Diem - WNY
About The Position
Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! This is a per diem position for Western New York, with a schedule of 1-2 days per week. The role involves providing assessments for newly referred patients, developing patient-centered care plans, and continually reassessing patient needs. The supervisor will also prepare progress summaries, supervise and train PCA/HHA staff, and participate in the on-call rotation for which a stipend is provided. The ideal candidate should have an empathetic approach to helping others and thrive in a fast-paced environment.
Requirements
- NYS Licensed Registered Professional Nurse
- Minimum of Associate's Degree required
- One year Nursing experience required

- Home care experience a plus
Responsibilities
- Provide assessment of newly referred patients and assess health needs in home care setting
- Work with patient to create a patient centered plan of care
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
- Provide supervision and training of PCA/HHA staff
- Assist with the on-call rotation (stipend provided)
