About The Position

Well-established and highly reputable home healthcare agency with 40 years of experience, striving to improve the quality of life for clients by delivering exceptional industry-leading in-home care. The agency has cared for close to 100,000 clients throughout the 5 boroughs of New York City and Westchester County. They are looking to add a hard-working team player to their training staff. This is a per diem, per-visit position. Compensation is paid per completed visit, not as a traditional hourly schedule. The hourly range shown is included because the job board requires a standard salary format and does not offer a “per visit” option. Actual earnings will vary based on the number and type of visits accepted. Special Touch currently pays $125 per initial visit and $100 per 6-month reassessment visit in Brooklyn, which represents a strong and competitive per-visit rate for comparable RN field work. Flexible fee-for-service opportunities are available now for qualified RNs seeking weekday field-based work in Brooklyn. Special Touch Home Care is currently seeking several experienced RNs for rewarding and flexible fee-for-service Registered Nurse positions within their licensed home care services agency (LHCSA). They believe clients deserve home health care delivered with compassion, excellence, and reliability. Field Supervisor RNs are responsible for conducting initial evaluations and reassessments of home care clients. This role offers the opportunity to build meaningful relationships with clients while making a positive impact on their day-to-day quality of life.

Requirements

• Current and valid NYS RN License
• Excellent written and verbal communication skills
• Minimum of 1 year of RN home care field experience required
• Excellent patient care and assessment skills
• Willingness to travel throughout Brooklyn, with a strong focus on Downtown Brooklyn, Flatbush, East New York, and surrounding service areas. Coverage areas include zip codes 11203, 11208, 11209, 11210, 11213, 11221, 11225, 11226, 11234, 11236, 11237, and 11239.

Responsibilities

• Conducting initial evaluations and reassessments of home care clients in the field.
• Working independently to build meaningful relationships with clients.
• Playing an important part in maintaining quality patient care in the home setting.
• Conducting supervisory visits to assess clinical needs, observe caregiver performance, and help ensure that caregivers are knowledgeable, competent, and properly trained.
• Internal case management and related documentation.
