About The Position

Southwoods Health at Home delivers essential healthcare services directly to patients closer to home. We are seeking dedicated Registered Nurses to manage patient care continuums from initial intake through discharge within our growing home health division.

Requirements

• Active Registered Nurse (RN) license in the State of Ohio.
• Current American Heart Association (AHA) BLS certification.
• Valid driver’s license with clean driving record and proof of current automobile insurance.
• Exceptional time management, clinical documentation accuracy, autonomous prioritization, and critical thinking skills.

Nice To Haves

• Bachelor of Science in Nursing (BSN) is preferred.

Responsibilities

• Develop, implement, and adjust comprehensive care plans tailored to meet individual home-health patient needs safely and effectively.
• Closely assess patient conditions, execute ordered interventions accurately, and document status changes with high precision.
• Provide regular progress updates to the multidisciplinary clinical team to support seamless goal management.
• Maintain patient confidentiality and strict adherence to protected health information (PHI) and HIPAA guidelines.
• Practice strong stewardship and cost-containment by utilizing clinical supplies and resources appropriately.
