RN Field Case Supervisor

About The Position
This is a true field position for a Registered Nurse (RN) who enjoys being out in the community, visiting 5-6 clients a day across Franklin County. The role involves opening new cases and completing supervisory visits for home care clients, spending approximately 95% of the time on the road. The ideal candidate is comfortable with technology, efficient, and provides warm, effective client visits while managing their own schedule and documentation.
Requirements
- Active Ohio RN license.
- Clean driving record with reliable transportation.
- Home care, home health, or hospice field experience strongly preferred.
- Comfortable with technology and timely, accurate documentation.
- Self-starter who manages their own route and calendar effectively.

Responsibilities
- Conduct start-of-care assessments and open new cases across various programs (private pay, Medicaid waiver, Franklin County Senior Options, VA, etc.).
- Complete 60- and 90-day supervisory visits to support caregivers and ensure client well-being.
- Manage visits efficiently with genuine warmth, keeping the schedule on track.
- Complete documentation on the same day.
- Serve as a clinical resource and point of contact for field staff and families.
