About The Position

The Hospice Field Registered Nurse (RN) Case Manager plays a vital role in providing compassionate, patient-centered end-of-life care in the comfort of patients' homes and care facilities. In this role, you'll coordinate and manage each patient's plan of care while focusing on comfort, dignity, and quality of life. Working alongside an interdisciplinary hospice team, you'll provide skilled nursing care, symptom management, patient and family education, and emotional support during one of life's most meaningful journeys. Requirements

  • Current, unrestricted RN license in the applicable state
  • Current CPR certification
  • Valid driver's license and reliable transportation with current auto insurance
  • Minimum of 2 years of nursing experience
  • Excellent assessment, communication, and time management skills
  • Ability to work independently while collaborating with an interdisciplinary team

Nice To Haves

  • Hospice or Home Health experience preferred
  • Hospice Case Management experience preferred
  • HomeCare HomeBase experience preferred

Responsibilities

  • Complete comprehensive nursing assessments and ongoing evaluations of hospice patients.
  • Develop, implement, and manage individualized hospice plans of care in collaboration with physicians and the interdisciplinary team.
  • Provide skilled nursing care focused on pain and symptom management, comfort, and quality of life.
  • Educate patients, families, and caregivers on disease progression, medications, symptom management, and end-of-life care.
  • Coordinate services with physicians, social workers, chaplains, hospice aides, and other healthcare professionals.
  • Monitor patient conditions and promptly communicate changes to the hospice physician and care team.
  • Supervise Hospice Aides and LPN/LVNs to ensure high-quality patient care.
  • Maintain accurate, timely clinical documentation in accordance with Medicare Conditions of Participation and hospice regulations.
  • Participate in interdisciplinary team meetings and contribute to ongoing patient care planning.
  • Provide compassionate support and guidance to patients and families throughout the hospice journey.
  • Ensure compliance with all federal, state, and agency policies and standards.
