About The Position

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, founded in 1982 as Joliet Area Community Hospice. They provide exceptional serious illness care, hospice care, and grief support for adults and children. The majority of their direct patient care is delivered in the field, with team members providing care in homes, nursing facilities, or assisted living facilities. They also operate a 24-bed inpatient unit in Joliet, and non-clinical staff are based out of their Joliet administrative offices. The RN Field Case Manager (Lake Forest Territory) is responsible, as a member of the interdisciplinary team, for providing skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers, consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.

Requirements

Current RN license
Graduate of accredited nursing program
Minimum of 2 years of nursing experience
Valid driver’s license and insured, reliable vehicle

Nice To Haves

At least 1 year in public health, home care, or hospice preferred

Responsibilities

Deliver skilled nursing care and hospice services in home and community settings
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications and treatments while monitoring patient symptoms and outcomes
Educate patients and caregivers on care, self-management, and disease progression
Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
Maintain accurate documentation and participate in on-call coverage as needed
