About The Position

This role involves performing an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. The RN Fellowship collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. They utilize leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • Less than 1 year med/surg experience, required.
  • PALS within 6 months of hire.

  • Bachelor’s Degree in Nursing, preferred.
  • BCLS, ACLS, preferred.

Responsibilities

  • Assesses the patient’s physiologic health status, including gastrointestinal, cardiovascular, respiratory, renal, and neurological health status. Determines mobility, sensory deficits, prostheses use, and skin condition, and adjusts the plan of care as appropriate. Assesses level of pain and pain management. Communicates and documents patient’s physiologic health status and plan of care.
  • Assesses patient’s psychosocial health status, including perception of medical/nursing care and expectations of care. Determines coping mechanisms, knowledge level, and ability to comprehend. Identifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction. Communicates and documents psychological status and care plan, support group, counseling services, and social services.
  • Identifies patient outcomes by developing criteria for measurement, identifying actual/potential patient problems, and assessing patient’s need for teaching based on psychosocial and developmental assessment. Develops patient outcome statement(s) and establishes individualized patient goals.
  • Formulates the plan of care and establishes priorities necessary to achieve expected outcomes. Coordinates the cost-effective use of supplies, equipment, and medication. Documents the plan of care and collaborates with physicians and other health team members.
  • Implements the plan of care by maintaining constant vigilance over patient care to ensure safety precautions/needs are followed. Exercises professional skills, reassesses patients as needed, and revises the plan of care. Correctly administers prescribed treatments and uses necessary equipment. Provides emotional support, applies scientific principles safely, timely, and efficiently. Makes accurate observations and reports/records them appropriately. Keeps accurate documentation of patient’s treatment, activity, and condition, as well as responses to interventions. Acts rapidly and effectively during emergencies. Provides a calm atmosphere, communicates effectively with family/caretakers. Participates in discharge planning and coordinates referrals. Provides discharge instructions.
  • Evaluates care provided for patient outcome, demonstrating ability to measure effectiveness and documenting the same. Performs variance analysis for performance improvement. Designs, implements, and evaluates systems to improve care in the unit. Keeps accurate documentation and uses appropriate methods of documentation.
  • Collaborates with other care team members in planning and carrying out the treatment regimen and provides direction to other members of the care team. Collaborates with the appropriate physician on the patient’s plan of care. Accurately interprets and implements treatment regimens. Assists the physician during treatments/procedures. Keeps the Patient Care Manager/designee and/or physician abreast of changes in patient’s condition/treatments. Uses clinical judgment in delegating assignments to LPN, Patient Care Technician, Support Care Associate, and Nursing Unit Secretary Associate.
  • Performs grade I-IV Pressure Injury Care, including preventive skin care measures. Applies simple dressings, maintaining aseptic technique. Applies warm & cold compresses and performs irrigations consistent with facility procedure. Performs other procedures related to skin care as necessary.
  • Administers medications correctly and safely, identifying medication action, dosage, side effects, and implications for the patient. Meets standard on medication administration examination. Demonstrates preparation of local solutions and IV drips. Administers and documents medication correctly. Administers IVP medication and IV standard infusions per department policy. Educates patients and significant others related to drug and food interactions.
  • Participates in patient and family education by explaining the patient’s condition, communicating assessment data, recognizing and utilizing teaching opportunities and resources. Provides for early discharge planning and appropriate referrals. Evaluates the effectiveness of teaching.
  • Performs related duties as required.
