Family Medicine at Richmond Clinic is a Federally Qualified Health Center (FQHC) providing comprehensive primary care to patients across the lifespan, including pregnant individuals, infants, children, adolescents, adults, and older adults. Patients are often moderately stable to moderately unstable, with complex medical conditions and significant social determinants of health (SDOH) needs. A substantial portion of nursing work occurs outside of scheduled provider visits. Nursing services include RN visits, telephone triage, obstetric intakes and support, medication administration, patient education, and transitions of care and RN care management. RN visits include assessment and management of acute and chronic conditions, medication and minor procedural care (ex: suture removals), chronic disease monitoring, and preventive follow-up. Nurses also support maternal-child health, substance use treatment, and care management through patient education and coordination. Care is delivered through a team-based, interdisciplinary model focused on whole-person care and improved patient outcomes.
