About The Position

Evernorth Workplace Care provides health care delivery, population health, and health coaching services. Its mission is to deliver proactive, personalized, and holistic patient care and coaching by using health data and insights to improve the health and wellness of clients’ employees and their families. Solutions are customized to organizational needs to support an affordable, predictable, and simple health care experience. The RN Diabetes / GLP-1 Educator - Health Coach provides virtual health coaching and education, primarily focused on diabetes and GLP-1 education. The role supports patients in setting and achieving evidence-based health goals, addressing barriers, improving clinical outcomes, and becoming active participants in their health. The coach also contributes to wellness activities and client health strategies, tracks clinical targets and outcomes, and may support training or mentorship of new employees. This is a virtual position performed from a dedicated home office in the contiguous United States, with a focus on patients in the Central and Mountain time zones. The home internet connection must be cable broadband or fiber optic, with speeds of at least 10 Mbps download and 5 Mbps upload. The anticipated annual salary is $77,500–$129,100, depending on relevant factors including experience and geographic location; the role is also anticipated to be eligible for an annual bonus plan.

Requirements

RN with an active, unrestricted license in the respective state and the ability to maintain it.
Compact or multistate license required, with the ability to maintain it; additional state licenses may also be required.
Certified Diabetes Educator.
Current ACLS/BLS/CPR/AED certification.
At least an Associate’s Degree in Nursing.
At least 3 years of RN experience in a hospital or provider’s office setting.
At least 1 year of experience in health coaching, health education, and health promotion.
High-speed internet and a quiet office space in a dedicated location.
High energy level and dynamic presentation skills.
Ability to serve as a positive role model by demonstrating healthy behaviors, with a passion for health improvement.
Ability to work independently and maintain a customer-centric focus.
Ability to collaborate proactively and professionally with clients and matrix partners.
Proven administrative abilities and strong computer and software application skills.
Adaptability and flexibility in a growing team.
Ability to cover multiple time zones if needed.

Nice To Haves

Bachelor’s Degree in Nursing is strongly preferred.
Behavioral health experience, including stress reduction.
Master’s degree in psychology or a related field.
Chronic disease management experience.
National Board-Certified Health & Wellness Coach (NBHWC) credential.
Motivational interviewing training or experience.
Additional languages, including Spanish.

Responsibilities

Provide customer coaching and support by video and telephone, focused on diabetes and GLP-1 education.
Perform stress, anxiety, and depression screenings.
Identify customer health education needs through targeted health assessment activities.
Refer customers to other Cigna Evernorth clinical programs or client-specific programs when clinically appropriate.
Collaborate with customers to establish health improvement plans, set personalized evidence-based goals, and support progress toward those goals.
Empower customers to participate actively in their health outcomes and assist them in overcoming barriers to better health.
Lead and support health and wellness promotional activities, including group coaching, wellness challenges, and health-related seminars.
Use biometric values and motivational interviewing techniques to collaborate with customers and improve clinical outcomes.
Collaborate with matrix partners to support the health and wellness strategy for aligned clients.
Understand and manage clinical targets and outcome measurements.
Develop action plans that drive clinical value for customers and clients.
Perform additional responsibilities within the scope of the employee’s certification or licensure.
Provide training or mentorship to new employees as needed.
