About The Position

This role involves supporting patients in the prevention and management of diabetes through individualized education, goal setting, and ongoing support. You will work collaboratively with physicians, providers, nurses, and other members of the healthcare team to provide comprehensive, patient-centered diabetes care. The role also involves partnering with patients and families to build the knowledge, skills, and confidence needed to successfully manage diabetes and improve overall health. Additionally, you will support the organization’s strong commitment to wellness and prevention, with opportunities to participate in health and wellness initiatives for employees and the communities served. RDNs will provide individualized medical nutrition therapy (MNT) for diabetes and other nutrition-related conditions across outpatient, inpatient, and long-term care settings. This is a collaborative rural healthcare team environment that values professional growth, teamwork, wellness, and community involvement.

Requirements

  • Bachelor’s degree in Dietetics (for RDN)
  • Associate’s degree in nursing (for RN)
  • Registered as a Dietitian - Academy of Nutrition and Dietetics (for RDN)
  • Registered Dietitian State Certification or License and some positions may require multiple state licenses (for RDN)
  • Registered Nurse (RN) licensed minimally in the state of MN. (for RN)
  • Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross.

Nice To Haves

  • Master's degree (for RDN)
  • Bachelor's degree in nursing (for RN)
  • Certified Diabetes Care and Education Specialist (CDCES) within five years of hire date (for both RN and RDN)

Responsibilities

  • Provide LOVE + MEDICINE while supporting patients in the prevention and management of diabetes through individualized education, goal setting, and ongoing support.
  • Work collaboratively with physicians, providers, nurses, and other members of the healthcare team to provide comprehensive, patient-centered diabetes care.
