This role involves supporting patients in the prevention and management of diabetes through individualized education, goal setting, and ongoing support. The specialist will work collaboratively with physicians, providers, nurses, and other members of the healthcare team to provide comprehensive, patient-centered diabetes care. They will partner with patients and families to build the knowledge, skills, and confidence needed to successfully manage diabetes and improve overall health. The role also supports the organization’s strong commitment to wellness and prevention, with opportunities to participate in health and wellness initiatives for employees and the communities served. For Registered Dietitians (RDN), this includes providing individualized medical nutrition therapy (MNT) for diabetes and other nutrition-related conditions across outpatient, inpatient, and long-term care settings. The position is part of a collaborative rural healthcare team that values professional growth, teamwork, wellness, and community involvement.
