About The Position

Provides patient service coordination for the surgical service line with the goal of navigating the pre-operative, hospital admission and post-acute phases of care. Coordinates the perioperative services with the patient, family, physicians, clinical team, and payer as needed. Complies with Enhanced Recovery After Surgery (ERAS) and other alternative payment model requirements for each surgical event if applicable. Identifies collaborator opportunities in the surgical services to achieve a superior patient experience with lower length of stay and cost.

Requirements

  • Bachelor of Science Nursing Required
  • Healthcare - Discharge Planning –or
  • Healthcare - Utilization Review -or-
  • Healthcare - Case Management Required
  • Healthcare - Elective Orthopedic Surgical Patients Required
  • RN Registered Nursing - California Board of Nursing Required
  • BLS Basic Life Support - American Heart Association Required
  • CCM Certified Case Manager - CCMC Commission for Case Manager Certification -or-
  • ACM Accredited Case Manager - ACMA American Case Management Association
  • Excellent patient clinical review and reporting skills as required.
  • A high degree of organizational skills, ability to set priorities, manage multiple demands and the ability to complete tasks under strict time lines is required.
  • Clear, thorough and concise documentation skills including attention to detail, proactive analysis and team collaboration.
  • Strong written and verbal communication skills with a focus on customer service.
  • Knowledgeable about Medicare and commercial contracts for acute and post-acute care.
  • Proficient in working between various IT systems.

Nice To Haves

  • Knowledge of CMS and CJR regulations preferred
