About Duke HomeCare & Hospice Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC. Job Summary The Care Coordinator is responsible for the assessment and coordination of primarily infusion referrals and patient device teaching along with coverage for home health and hospice coordination for the hospitalized patient. The coordinator reviews admission information and assesses the patient for appropriateness of home care services. The primary focus of this role is coordinating Infusion referrals for adult and pediatric patients within Duke University Hospital. The coordinator may provide infusion device teaching for the infusion device used in the home. The CCC functions as an integral part of an interdisciplinary team, responsible for providing program specific education, responding to customer requests and concerns, and interfacing with referral sources, organizations, government agencies and third party payors.
