About The Position
As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life. This role involves conducting initial assessments, establishing plans of care, and providing prescribed treatments. You will coordinate the delivery of 24-hour care, counsel and guide individuals and families towards self-help in recognizing and solving physical, emotional, and environmental problems. Additionally, you will conduct regular reassessments of each patient and maintain complete and accurate clinical documentation, while also supervising and serving as a resource to all home care staff.
Requirements
- Nursing Diploma required
- License and current registration to practice as Registered Professional Nurse in New York State
- Current NYS Driver’s license
- BLS - Basic Life Support - American Heart Association (AHA)
- RN - Registered Nurse - New York State Education Department (NYSED)
Nice To Haves
- ASN preferred
- Bachelors degree of Science in Nursing preferred
- 1 year of experience working with a frail or elderly population preferred
- 6 months- 1 year experience conducting UAS-NY Community Assessment preferred
- Availability of Motor Vehicle
