About The Position

As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life. This role involves conducting initial assessments, establishing plans of care, providing prescribed treatments, and coordinating 24-hour care. You will also counsel and guide individuals and families towards self-help in recognizing and solving physical, emotional, and environmental problems, and conduct regular reassessments of each patient while maintaining complete and accurate clinical documentation. Additionally, you will supervise and serve as a resource to all home care staff.

Requirements

• Nursing Diploma or ASN
• Registered RN in State of NY
• NY driver's License
• BLS - Basic Life Support - American Heart Association (AHA)
• RN - Registered Nurse - New York State Education Department (NYSED)

Nice To Haves

• BSN
• 1 year of experience working with a frail or elderly population
• Availability of Motor Vehicle
• 6 months- 1 year experience conducting UAS-NY Community Assessment

Responsibilities
