About The Position

Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components. The job will start at 2 to 3 8-hour shifts per week. By the summer of 2026 it will become full time, (40 hours) M-F 8:00am to 4:30pm.

Requirements

• Bachelor's Degree, required.
• Registered nurse in the State of Ohio, required.
• Effective customer service skills.
• Good multiplatform communication skills (oral, written, presentation).
• Ability to assist in conflict resolution.
• Problem-solving and interpersonal skills required for interaction with contacts required.
• Three years of registered nurse experience, required.
• OCCASIONALLY: Standing, Walking
• FREQUENTLY: Decision Making
• CONTINUOUSLY: Audible speech, Computer skills, Hand use: grasping, gripping, turning, Hearing acuity, Interpreting Data, Peripheral vision, Problem solving, Repetitive hand/arm use, Seeing – Far/near, Sitting

Nice To Haves

• Masters Degree in Nursing, preferred.

Responsibilities

• Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
• Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum.
• Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
• Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians.
• Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
• Develops clinical protocols, policies, and procedures including community education and public relations.
