About The Position

This is a full-time position requiring 36 or 40 hours per week. The schedule involves working either 7:00 PM - 7:00 AM or 11:00 PM - 7:00 AM, with an every-other-weekend rotation and holiday coverage. The role involves completing home assessments, planning and delivering nursing care, performing OASIS assessments, developing care plans, coordinating interdisciplinary care, educating patients and families, documenting care, supporting quality outcomes, participating in team meetings and quality improvement activities, maintaining professional growth, precepting students, and providing excellent customer service. Travel within an assigned territory is required, with occasional assistance outside the usual area. Other duties may be assigned.

Requirements

  • Registered Nurse with an Associate Degree in Nursing (BSN preferred).
  • Licensed and in good standing with the New Hampshire Board of Nursing.
  • At least one year of recent acute care experience.
  • Certified in BLS/CPR.
  • Able to meet driving, insurance, and background requirements.
  • Skilled in patient assessment, care planning, and communication.
  • Comfortable working independently in the home setting while collaborating with an interdisciplinary team.
  • Familiar with OASIS, home health reimbursement, and EMR systems (Allscripts preferred).
  • Compassionate, adaptable, and committed to delivering high-quality, patient-centered care.

Responsibilities

  • Complete initial and ongoing home assessments, including full physical evaluations and medical history reviews.
  • Use the nursing process to plan, deliver, and evaluate skilled nursing care.
  • Perform accurate OASIS assessments and ensure documentation reflects patient status and care needs.
  • Develop individualized care plans with measurable goals, involving patients and families in all planning and revisions.
  • Coordinate interdisciplinary care, communicating significant changes to physicians and obtaining necessary orders.
  • Teach and counsel patients and families on self-care techniques, disease management, and safety.
  • Prepare timely, accurate clinical notes and maintain strong communication with the care team.
  • Support quality outcomes, patient satisfaction, and appropriate utilization of visits.
  • Participate in staff meetings, case conferences, and quality improvement activities.
  • Maintain professional growth through continuing education and annual mandatory training.
  • Precept students when requested and provide excellent customer service.
  • Travel within assigned territory, occasionally assisting outside the usual area.
  • Perform other duties as assigned.
