Requirements

  • Graduate of an accredited RN college, BSN Preferred
  • Active Registered Nurse (RN) license in the State of Ohio without any board actions.
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage

Nice To Haves

  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to obtain certification.

Responsibilities

  • Plans, coordinates, and delivers patient care using the nursing process, including assessment, planning, intervention, implementation, and evaluation, while collaborating effectively with patients, families, and the interdisciplinary team to maintain professional nursing standards.
  • Performs comprehensive patient and family assessments, addressing physical, emotional, psychosocial, and spiritual needs.
  • Develops, implements, and regularly updates individualized, patient-centered care plans with measurable goals in collaboration with patients and families to ensure quality outcomes.
  • Reviews and revises the plan of care, including interventions, medications, and visit frequency, to meet patients' changing needs.
  • Assesses changes in patient condition, communicates findings to the physician, and assists in determining appropriate levels of care and continued eligibility for hospice services.
  • Participates actively in hospice admissions, interdisciplinary team (IDT) meetings, and care conferences to coordinate patient care and services.
  • Documents physician orders accurately in the electronic health record (EHR) to support safe, timely, and effective care.
  • Supervises, educates, and evaluates Personal Care Specialists, volunteers, and other caregivers to ensure care is delivered in accordance with the established plan of care.
  • Completes timely, accurate documentation of assessments, interventions, and changes in patient status at the point of care, synchronizing all records in the EHR according to Ohio's Hospice standards to support continuity of care.
  • Provides clear and timely handoff communication, including SBAR and other standardized communication methods, to ensure safe transitions and continuity of patient care.
  • Assists other departments and supports organizational needs as requested to ensure comprehensive, high-quality care for all Ohio's Hospice patients.
  • Participates in Continuous Quality Improvement (CQI) initiatives to enhance patient outcomes and organizational performance.
  • Serves as a preceptor and mentor for newly hired clinical staff, fostering professional development and successful integration into the hospice care team as assigned.
