This role involves monitoring the Electronic Health Record nurse pool, responding to patient needs, and triaging calls, messages, and in-basket tasks. The position requires prioritizing urgent matters, scheduling appointments, and utilizing nursing judgment and triage guidelines to meet patient health needs. The RN will assess complex patient needs, ensure appropriate transitions of care, and collect and assess patient data. Responsibilities include triaging and scheduling visits, providing clinical support, answering phone calls and electronic messages promptly, and educating patients about community services. The role also involves collaborating with the interdisciplinary care team, greeting and escorting patients, obtaining vital signs, performing medication reconciliation, assisting clinicians, and resolving patient concerns. Participation in practice meetings and continuous learning about community resources is expected. The RN must maintain patient rights, confidentiality, and provide clerical support as needed. The ability to float to other practices and verify/implement clinician orders is also required. This role also handles triaging walk-in patients and maintaining communication for continuity of care.
