The RN Clinical Supervisor is responsible for clinical oversight, quality assurance, and operational execution of home health services for assigned nursing staff. This role ensures that client care delivery, clinical documentation, and staff performance meet all applicable state regulations, CMS Conditions of Participation (CoPs), payer requirements, and accreditation standards. Reporting to the Executive Director, the RN Clinical Supervisor serves as the frontline clinical leader, directly supervising Case Managers, SOC/ROC RNs, LPNs, and other assigned clinicians. This position integrates clinical leadership, quality management, regulatory compliance, and performance accountability to ensure safe, effective, and efficient client care across the full episode lifecycle. In addition to clinical and operational leadership, the RN Clinical Supervisor is a visible representative of CareAparent and is responsible for ensuring their team consistently reflects The CareAparent Way. This includes setting expectations for professionalism, communication, service excellence, and compassionate, client-centered care. The Supervisor reinforces that how care is delivered is as important as the clinical outcome, holding their team accountable to both performance standards and the CareAparent brand experience. This role requires a hands-on, data-driven leader who proactively identifies clinical risk, drives quality outcomes, and ensures staff accountability, and fosters a culture aligned with CareAparent’s mission, values, and commitment to delivering The Art of Care.

Requirements

  • Associate Degree (BSN preferred)
  • 3+ years of clinical leadership or supervisory experience
  • 3+ years of home health experience
  • Active RN license in the state of Minnesota (unencumbered)
  • Valid driver's license with an insured, reliable vehicle
  • All new hires must pass a background check prior to employment.

Responsibilities

  • Directly supervise, coach, and evaluate assigned clinical staff to ensure delivery of safe, high-quality client care.
  • Conduct field supervision visits, competency validation, and performance evaluations in accordance with regulatory requirements.
  • Provide structured onboarding, orientation, and ongoing clinical education.
  • Hold staff accountable for: Clinical performance, Documentation quality and timeliness, Productivity standards, Professional conduct.
  • Initiate and manage corrective action and performance improvement plans as needed.
  • Actively model and enforce The CareAparent Way, ensuring all assigned team members consistently deliver a high-touch, responsive, and professional client experience aligned with CareAparent standards.
  • Oversee the full client episode lifecycle, including: Start of Care (SOC), Resumption of Care (ROC), Recertification, Change in Condition, Discharge planning.
  • Ensure care is delivered in accordance with: Physician/NPP orders, Individualized Plan of Care, Evidence-based practice standards.
  • Review referrals to confirm appropriateness and service capability.
  • Support clinical triage and escalation for high-risk clients.
  • Participate in case conferences and interdisciplinary team (IDT) meetings.
  • Own ongoing clinical record audits and documentation quality across assigned staff: OASIS accuracy and integrity, Documentation completeness and timeliness, Plan of Care compliance.
  • Own and ensure compliance with: CMS Conditions of Participation (42 CFR 484), State of Minnesota home care regulations, Accreditation standards (e.g., The Joint Commission).
  • Identify trends, gaps, and risk areas; implement targeted remediation and staff coaching.
  • Partner with Compliance and QAPI to support: Internal audits, Survey readiness, Corrective action plans (CAPs).
  • Ensure appropriate use and proficiency in EMR systems.
  • Maintain current knowledge of: CMS regulations, State home care statutes, Payer requirements.
  • Ensure adherence to all clinical, documentation, and operational regulatory standards.
  • Investigate and respond to: Incidents, Complaints, Medication errors, Adverse events.
  • Oversee infection prevention practices and communicable disease management.
  • Ensure staff compliance with required screenings, competencies, and certifications.
  • Ensure clinical decisions and team behaviors consistently align with both regulatory standards and CareAparent service expectations.
  • Own daily clinical operations and team performance, including: Staffing coverage, Visit scheduling support, Caseload distribution.
  • Monitor and manage productivity and efficiency metrics.
  • Collaborate with intake, scheduling, and operations to ensure: Timely admissions, Continuity of care.
  • Optimize workflows to improve efficiency and reduce delays in care delivery.
  • Provides after-hours clinical support and decision-making as needed.
  • Assigns rotating on-call coverage to team and serves as backup as needed.
  • Own and drive performance across key performance indicators (KPIs), including: Staff Productivity (i.e., availability and points), Documentation timeliness, OASIS quality outcomes, Hospitalization rates, HHVBP performance measures.
  • Utilize data to drive accountability and performance by: Implementing performance improvement, Identifying trends and risks, Guiding decision-making and accountability.
  • Escalate operational or clinical risks with actionable solutions.
  • Serve as the clinical leader and standard-bearer for both clinical excellence and the CareAparent client experience.
  • Represent CareAparent in all internal and external interactions, ensuring professionalism, responsiveness, and alignment with brand expectations.
  • Facilitate communication between clinicians, leadership, and interdisciplinary teams.
  • Participate in leadership meetings, QAPI initiatives, and agency planning.
