About The Position

The Care Transitions Clinical Supervisor provides support and leadership to Recuperative Care nursing and support staff utilizing reflective proactive, staff development, support and oversight to build and model a strengths-based, client centered culture. The Recuperative Care Clinical Supervisor coordinates the administration medical services to clients, including assessing, screening, educating and treating patient conditions. The Supervisor oversees all medication management workflows, practice, policies, monitoring and implementation of patient self-administration of medications to minimize risk.

Requirements

  • Current license as an RN in the state of California, graduate of an accredited school of nursing.
  • Must have current BLS Certification or willingness to obtain within 90 days from date of hire.
  • Four plus year’s related experience in clinical experience with diverse adult population preferred; OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
  • Two years of leadership experience supervising of a team of Licensed Vocation Nurses and support staff.
  • Commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.

  • Educate staff, patients and/or families and to provide instruction on proper care and treatment in addition to best practices and adherence to certifying and regulatory standards of care.

  • Ability to establish and maintain positive, professional relationships.
  • Computer literacy including use of email, Microsoft Word and Excel and Electronic Health Records, preferably experience with EPIC.
  • Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.
  • Participation in an on-call rotation, including responding to urgent matters outside of regular business hours.
  • Employees must be reachable, able to respond promptly, and take appropriate action as needed.
  • Communicate clearly and concisely, both verbally and in writing.
  • Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.
  • Ability to research best practices and most current regulatory and certifying agency requirements in order to write program policies and processes.

  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Must be willing to successfully complete the credentialing process.

Responsibilities

  • Works as part of a multidisciplinary team with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring.
  • Trains, coaches, and mentors nursing, support staff and new hires in patient support duties, medication management and monitoring
  • Ensures nurses and support staff are trained and abide by organizational policies, standing operating procedures and guidelines.
  • Performs RN duties for patients receiving medical respite (recuperative care) including taking medical histories, completing assessments, reconciling medications and performing other nursing functions according to scope of practice.
  • Provides education and oversight to ensure nurses and support staff follow best practices in client care and adheres to certifying and regulatory standards.
  • Oversees training, monitoring, auditing and compliance of medication management; medication intake, logging, entry into electronic health record, documentation of medication changes, narcotic workflows, medication disposals and discharges, and patient medication calls.
  • Works directly with Clinical Risk Manager to identify errors, do root cause analysis, and develop error prevention workflows to reduce risk.
  • Trains, coaches and evaluates staff in medication management performance, errors and prevention.
  • Remains actively involved in recruiting, interviewing and hiring all nursing and support staff.
  • Provides education and best practice updates at all staff meeting and during in-services.
  • Assesses and develops strategies to address knowledge and skill gaps of nursing and support staff including providing performance improvement plans as needed.
  • Provides adequate orientation, professional opportunities and study time to remediate gaps in necessary knowledge skill by coordinating with nursing supervisor.
  • Writes Recuperative Care policies, standing operating procedures and guidelinesusing best practice standards.
  • Builds confidence and skills of nursing and support staff.
  • Provides input for evaluation and administrative data of nursing and support staff including annual leave, annual review, disciplinary action, etc.
  • Audits and ensure timely and accurate data input and utilization of the electronic medical records.
  • Ensures implementation of quality improvement strategies.
  • Provides routine surveillance of potential client care and compliance issues.
  • Assists in implementing agency outcomes and interventions.
  • Maintains relationships based on trust, support and growth with team colleagues and community members.
  • Ensures nursing and support staff utilizes professional principles and concepts of motivational interviewing, coaching and reflective practice to model therapeutic relationships, client centered care and solution focused approaches.
  • Provides regular case conferences with Recuperative Care Director and and conducts team meetings with nursing and support staff.
  • Reviews patient electronic health records and medical charts.
  • Encourages client and staff general wellbeing through health promotion.
  • Refers patients to higher level of care and specialists.
  • Keeps accurate records for reporting to various funding and inspecting agencies.
  • Monitors nursing and support staff practices for adherence to Joint Commission standards and adherence.
