About The Position
The RN Clinical Manager is responsible for leading clinical operations within a home health setting, ensuring high-quality, compliant, and patient-centered care. This role oversees clinical staff, coordinates care delivery, and drives performance-improvement initiatives to enhance patient outcomes and operational efficiency. You’ll serve as a key leader, supporting interdisciplinary collaboration while ensuring adherence to regulatory standards and agency policies.
Requirements
- Current Registered Nurse (RN) license (required)
- Minimum 2+ years of home health management experience
- Strong knowledge of home health regulations and compliance standards
- Current CPR certification
- Valid driver’s license and auto insurance

- Experience with HomeCare HomeBase (HCHB) preferred
Responsibilities
- Lead and oversee clinical operations to ensure safe, high-quality patient care in compliance with state and federal Conditions of Participation (CoPs)
- Supervise and support clinical staff, ensuring appropriate care planning, reassessment, and continuity of services
- Coordinate and collaborate with interdisciplinary teams to optimize patient outcomes
- Monitor and manage the QAPI program, implementing corrective actions as needed
- Ensure compliance with Medicare, Medicaid, and other third-party payer requirements
- Identify operational or clinical issues and communicate findings to leadership with actionable recommendations
- Promote infection control practices and adherence to safety standards
- Support staff development, training, and ongoing education
- Participate in interdisciplinary group (IDG) meetings and performance improvement initiatives
- Foster a culture of accountability, communication, and clinical excellence
