About The Position
AccordCare Home Health, a licensed Medicare-certified agency, is seeking a customer service-driven and detail-oriented RN Clinical Manager to join its Maitland team. This is a full-time, in-office role with limited field case management as needed. The RN Clinical Manager is responsible for assessments, coordination of care, communication with referral sources, and supporting field clinicians to ensure high-quality patient care. The role requires screening through the Care Provider Background Screening Clearinghouse.
Requirements
- Current Florida RN license.
- Level II Background Screening.
- CPR certification.
- At least 1 year of Medicare Home Health Case Management experience.
- At least 1 year of Medicare OASIS experience.
- Excellent time management and organizational skills.
- Strong communication and customer service abilities.
- Must have Medicare home health experience.
Nice To Haves
- Experience with Kinnser / WellSky Medicare.
Responsibilities
- Perform case management, oversight, and coordination of care for all skilled disciplines.
- Coordinate patient admissions with other members of the healthcare team.
- Provide nursing care and treatment to home care patients according to the plan of treatment and approved authorizations.
- Supervise all field staff on assigned cases.
- Support field clinicians and communicate with referral sources.
