About The Position

Preferred Home Care of Florida is seeking a Home Health - Clinical Manager. The ideal candidate will have previous Home Health experience with Medicare, Joint Commission and PDGM training. This is a growth opportunity leading to the potential Assistant Director of Nursing position.

Requirements

• Excellent communication skills
• Knowledge of Medicare OASIS
• Knowledge of Medicare Discipline Frequency
• Computer Skills a must (EMR- AXXESS System Core)
• Knowledge of State, Medicare and Joint Commission guideline
• Ability to audit OASIS for documentation and Discipline Visit notes
• Must make patient care a top priority
• Must have a positive attitude and be a team player
• Current Nursing License in the State of Florida
• Nursing degree from accredited school

Nice To Haves

• Previous Home Health experience with Medicare, Joint Commission and PDGM training
• Axxess System – Experience is a MUST!!!!
• Medicare Oasis proficiency within a Home Health Agency and QA experience
• Familiar with ICD-10 coding
• Former Field Nurse in Home Health and/or Case Management experience

Responsibilities

• Audit OASIS for documentation and Discipline Visit notes
• Generate Plan of Care as per conditions of participation
