Requirements

  • Registered Nurse with an active, current state license.
  • Two years of clinical nursing experience.
  • Graduate of National League for Nursing accredited school of nursing.
  • Two years management or supervisory experience within the past five years in a home health agency, primary care clinic, or health facility.
  • Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
  • Complies with accepted professional standards and practice.
  • Complies and maintains current CPR certification for healthcare providers.
  • Demonstrates excellent observation, verbal and written communication skills.
  • Licensed driver.

Nice To Haves

  • BSN degree is preferred.

Responsibilities

  • Receives case referrals and reviews patient information to determine home care needs, assigning appropriate clinicians.
  • Reviews and evaluates each case by examining services provided by clinicians, discussing impressions, and guiding them for effective performance and quality care.
  • Reviews patient's clinical diagnosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.
  • Attends case conference meetings to facilitate care coordination.
  • Conducts quarterly record reviews and communicates findings and recommendations.
  • Assists in the screening and interviewing process of new personnel and makes recommendations for employment.
  • Assists in the orientation of new personnel.
  • Assists in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, strategic goals, and objectives.
  • Complies with accepted professional standards and principles.
  • Participates in public relations and community activities to promote the Organization's role.
  • Promotes a customer service orientation.
  • Performs other duties as delegated by the Assistant Clinical Director/Clinical Director.
