Requirements

Registered Nurse with an active, current state license.
Two years of clinical nursing experience.
Graduate of National League for Nursing accredited school of nursing.
Two years management or supervisory experience within the past five years in a home health agency, primary care clinic, or health facility.
Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
Complies with accepted professional standards and practice.
Complies and maintains current CPR certification for healthcare providers.
Demonstrates excellent observation, verbal and written communication skills.
Licensed driver.

BSN degree is preferred.

Responsibilities

Receives case referrals and reviews patient information to determine home care needs, assigning appropriate clinicians.
Reviews and evaluates each case by examining services provided by clinicians, discussing impressions, and guiding them for effective performance and quality care.
Reviews patient's clinical diagnosis, medications, procedures, and clinical course.
Assists clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.
Attends case conference meetings to facilitate care coordination.
Conducts quarterly record reviews and communicates findings and recommendations.
Assists in the screening and interviewing process of new personnel and makes recommendations for employment.
Assists in the orientation of new personnel.
Assists in the planning, implementation, and evaluation of in-service and continuing education programs.
Assists in the formulation, revision, implementation, and evaluation of policies and procedures, strategic goals, and objectives.
Complies with accepted professional standards and principles.
Participates in public relations and community activities to promote the Organization's role.
Promotes a customer service orientation.
Performs other duties as delegated by the Assistant Clinical Director/Clinical Director.
