About The Position

Mountain Pacific is seeking a Registered Nurse (RN) to join their team as a Clinical Field Reviewer. This remote position requires the candidate to reside in Missoula or Ravalli County, Montana, and involves conducting in-home nursing assessments for the Medicaid CFCS/PCS program. The role focuses on assessing members' functional abilities and their need for assistance with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL). The team works collaboratively with provider agencies to ensure services are delivered as authorized and aims to help members remain independent in their homes with caregiver support. The ideal candidate will have experience with home visits, a passion for helping others, and the ability to work independently in the field, setting their own schedule while meeting deadlines.

Requirements

  • Reside in Missoula or Ravalli County, Montana.
  • Current Montana Registered Nursing (RN) license.
  • Reliable home internet.
  • Ability to travel by personal car to locations throughout Missoula and Ravalli Counties.
  • Experience with home visits.
  • Passion for helping others.
  • Ability to work independently in the field.
  • Ability to set your own schedule and meet your timelines.

  • Experience with home health, hospice, or long-term care.

Responsibilities

  • Conduct person-centered in-home nursing assessments focused on a member’s functional abilities and their needs for ADL and IADL assistance.
  • Complete assessments initially, annually, and as needed if a change in condition occurs for members of all ages.
  • Work closely with provider agencies across the state to ensure that services are being delivered to members as authorized following policies set forth by the State of Montana DPHHS.
  • Empower members to become participants in their care through active listening and planning with their input.
  • Provide care coordination.
  • Practice cultural humility with each client.
  • Utilize evidence-based practice as a foundation for interventions.
