About The Position

Mountain Pacific is seeking a self-motivated, organized Clinical Field Reviewer with good communication and nursing documentation skills to join their team. This remote position involves completing in-home nursing assessments for eligible members in Eastern Montana for the Medicaid CFCS/PCS program. The role focuses on person-centered assessments of members' functional abilities and needs for ADL and IADL assistance, aiming to help members remain independent in their homes with caregiver support. The Clinical Field Reviewer will work closely with provider agencies to ensure services are delivered as authorized, provide education, empower members in their care, offer care coordination, practice cultural humility, and utilize evidence-based practices.

Requirements

Valid driver’s license
Reliable home internet
Current Montana Registered Nursing license
Ability to travel by personal car to locations throughout Eastern MT

Responsibilities

Completing in-home nursing assessments of eligible members living in the areas where they reside.
Conducting person-centered in-home nursing assessments focused on a member’s functional abilities and their needs for ADL and IADL assistance initially and annually and as needed if a change in condition occurs for members of all ages.
Working closely with provider agencies across the state to ensure that services are being delivered to members as authorized following policies set forth by the State of Montana DPHHS.
Providing education.
Empowering members to become participants in their care through active listening and planning with their input.
Providing care coordination.
Practicing cultural humility with each client.
Utilizing evidence-based practice as a foundation for interventions.
