Every role is vital in advancing medicine and delivering the best care experience. This full-time position is in the PACE Community Care department, located at 300 Ocean Avenue – Revere.

Requirements

Graduate of an accredited RN program; BSN preferred.
Current Massachusetts license in nursing.
Current BLS certification required.
Exceptional triage assessment skills.
Ability to communicate assessment findings efficiently, effectively, and accurately in verbal and written formats.
Must complete a background check before beginning employment.

Responsibilities

Completing the Minimum Data Set (MDS) with prospective PACE enrollees.
Completing ongoing annual renewals of all enrollees in the program.
Reviewing the profile, sales plan, and medical record of potential enrollees.
Assessing medical records for documented diagnoses, medications, and physical and mental status.
Contacting potential enrollees to explain the purpose of the visit and schedule appointments.
Visiting potential enrollees to gather information, validate medical records, and complete the MDS.
Signing the completed MDS and having a provider review and co-sign it.
Entering MDS information into the Virtual Gateway.
Entering home care referrals into EPIC upon collaboration with the enrollment team for the enrollment date.
Reviewing the medical record of participants due for renewal one month prior to their anniversary date, in accordance with the MDS due date log.
Collaborating with the primary nurse to validate information gathered from the medical record.
