About The Position

Hospice of the West is seeking a Registered Nurse (RN) with strong clinical knowledge and supervisory experience to lead a team of clinicians as their Clinical Director. This role involves steering the interdisciplinary team of RNs, Aides, Social Workers, and Chaplains to provide care for patients and families. The Clinical Director collaborates with the Program Director, Hospice Physician, and Community Liaisons to manage office operations, drive patient growth, and ensure compliance with Federal, State, ACHC, and Hospice of the West guidelines. The ideal candidate is a strong leader with a drive to succeed, faith in their team, and high emotional intelligence. The schedule is Monday - Friday, 8 am - 5 pm, with an on-call rotation included. The territory is Phoenix.

Requirements

  • Registered Nurse with a current state license to practice as a professional nurse.
  • A minimum of 3 years experience as a nurse.
  • Including 1 year of full time experience in providing direct patient care in a hospice, home health or oncology setting.
  • Including 1 year of supervisory experience in hospice or home health care preferred.
  • Demonstrated ability to manage all aspects of terminal care with expertise in crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
  • Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family needs.
  • Computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

  • Bachelor's degree in nursing or health care preferred.

Responsibilities

  • Coordinate and oversee a spectrum of patient services, ensuring alignment with established hospice program policies.
  • Provide guidance and support to clinicians, fostering continuous enhancement of hospice care services.
  • Conduct chart audits, overseeing medical record upkeep for compliance with regulations, certification standards, and ethical care benchmarks.
  • Participate in crafting orientation and in-service training for nursing & clinical staff.
  • Evaluate and uplift clinical staff performance during probation and annually, offering teaching and counseling.
  • Lead interdisciplinary team gatherings to refine patient/family Care Plans, encouraging insight exchange, challenge resolution, and professional camaraderie.
