The RN Clinical Case Manager ensures supervision of all home care personnel, as assigned, and enforces/demonstrates compliance with State/Federal regulations. This role ensures effective communication to coordinate the implementation of each patient’s Plan of Care, and provides instruction, training, and re-education on clinical skills/procedures when necessary. The RN Clinical Case Manager is responsible for the direct supervision of field LVNs/LPNs/RNs per State guidelines, and provides support, guidance, and education to patients/patient families and/or caregivers to maintain optimal physical and emotional wellbeing. They communicate all occurrences and other pertinent information to the Clinical Supervisor/Director of Clinical Services and ensures any observed or suspected child/adult abuse, neglect, or exploitation is reported to proper authorities as mandated as a health professional. This role collaborates with the Clinical Supervisor and Director of Clinical Services to resolve client/clinical concerns in a timely manner. In the office, the RN Clinical Case Manager develops and implements the treatment plan for each patient under the direction of a physician, completes all clinical documentation in a timely manner, including admissions, recertification’s, and resumptions of care, ensuring accuracy and compliance with agency policies and State regulations. They review clinical documentation to ensure it is complete and reflects skilled services to meet payer source requirements for reimbursement. Additionally, they interview and evaluate potential clinical field staff to ensure competency, comply with and participate in continuing quality improvement by reporting incidents, injuries, and infections, and participate in agency-sponsored continuing education to maintain clinical competency.
