The RN Clinical Case Manager - Bilingual is responsible for ensuring the supervision of all home care personnel, enforcing compliance with State/Federal regulations, and ensuring effective communication to coordinate the implementation of each patient’s Plan of Care. This role involves instructing, training, and re-educating on clinical skills/procedures when necessary, and providing direct supervision of field LVNs/LPNs/RNs per State guidelines. The position also involves providing support, guidance, and education to patients, their families, and caregivers to maintain optimal physical and emotional well-being. The RN Clinical Case Manager communicates all occurrences and pertinent information to the Clinical Supervisor/Director of Clinical Services and ensures mandated reporting of any suspected abuse, neglect, or exploitation. Additionally, this role collaborates with the Clinical Supervisor and Director of Clinical Services to resolve client/clinical concerns. In the office, the RN Clinical Case Manager develops and implements treatment plans under physician direction, completes all clinical documentation accurately and in a timely manner, and reviews documentation to ensure it meets payer source requirements. The role also includes interviewing and evaluating potential clinical field staff for competency, and participating in quality improvement initiatives. The RN Clinical Case Manager supports the company's mission, ethics, and goals, representing the company professionally and adhering to all company policies and procedures.
